Like all medical practices, we operate under strict HIPPA rules. Your privacy is one of our greatest concerns. Therefore, we are striving to make our web presence a secure as possible experience for you. At this time we do not have secure email, so we ask you not to put personal patient information into an email. We cannot discuss specific patient conditions securely over email.
Our forms are all available in the left-hand column in PDF format. To download, you must have Adobe Acrobat Reader. You may download the Adobe Reader by visiting www.adobe.com . All registration forms must be completed yearly and signed by a legal guardian.
At Rainbow Pediatrics we work hard to meet your needs and to see you as soon as possible. We try to keep space open for our patients to be seen on the same day if needed. For this reason, we ask that you please cancel your appointment as soon as you realize that you are not going to be able to make your scheduled appointment. We try hard to stay on time, but please be aware that an emergency can derail the schedule. Because of this, we ask you to try to be on time for your appointment. If you are more than 20 minutes late we may ask you to reschedule your appointment.
If you miss three appointments without calling to cancel or reschedule, you will be sent a letter asking you to either come to your appointments in a timely fashion or cancel as soon as possible. If you continue to miss appointments, you will be asked to find another health care provider.
